refactor: narrow mypy exclusions for meta_reference

Narrowed mypy exclusions from entire meta_reference/ directory to only
specific files that still have type errors. The responses/ subdirectory
(6 files) now passes full mypy type checking.

Fixed types.py by adding type annotations and strategic type ignores
for mypy's confusion between similar discriminated unions
(OpenAIResponseInputTool vs OpenAIResponseTool).
