docs: Add Reranking API RFC and related updates 
- Added `RFC-0003-rerank-api.md` as an initial draft.
- Uploaded the Reranking API flowchart.
- Fixed image path for the reranking flowchart.
- Updated RFC-0003 with refinements after internal review.
- Fixed broken links in the RFC document.
